济南市住房和城乡建设局 关于开展2022年建筑业企业安全生产许可证动态考核的通知

各区县住房城乡建设局，济南新旧动能转换起步区管委会建设管理部、济南高新区管委会建设管理部、市南部山区管委会规划发展局，各有关单位：
为加强建筑施工企业安全生产许可证动态监管，促进企业提高和完善安全生产条件，根据《山东省建筑施工企业安全生产许可证动态考核办法》要求，结合我市实际，确定自即日起开展2022年全市建筑施工企业安全生产许可证动态考核工作。现将有关事宜通知如下：
一、考核范围
对全市建筑工程施工企业按照不少于20%的比例进行抽查考核，重点核查《安全生产许可证》2023年到期的建筑施工企业、近两年来发生过生产安全事故的施工企业、建筑工程质量安全辅助巡查发现重大隐患问题涉及的建筑施工企业。（考核企业名单见附件1）
二、考核内容
（一）安全生产责任制建立落实情况，安全生产规章制度、操作规程制定落实情况；
（二）企业安全生产管理机构设置及安全专职管理人员配备情况；
（三）企业主要负责人、项目负责人、专职安全生产管理人员、特种作业人员持证上岗及配备情况；
（四）企业管理人员和现场作业人员年度安全培训及考核情况；
（五）企业安全生产资金投入情况（包括企业保证安全生产投入的管理办法或规章制度、年度安全资金投入计划及实施情况）；
（六）企业依法缴纳工伤保险情况；
（七）企业职业危害防治措施和标准安全防护品配置情况；
（八）企业危险性较大的分部分项工程及重大危险源监控、预防措施和应急预案的设置情况；
（九）企业生产安全事故应急救援组织、人员、设备的配置及救援演练的设置演练情况；
（十）企业安全生产风险分级管控和隐患排查治理双重体系建设情况。
三、考核方式
（一）自查阶段。即日起至9月20日前，全市建筑施工企业根据《山东省建筑施工企业安全生产许可证动态考核办法》《建筑施工企业安全生产许可证核发办事指南》及本次考核内容逐项认真展开自查，对存在的问题及时整改到位。考核范围内的企业将自查报告（盖章PDF版）发送邮箱axz6666@163.com。
（二）一般安全生产条件核查。考核范围内的企业登陆济南市住房和城乡建设局网站“济南市智慧住建平台”平台进行填报，截止日期11月30日（详见附件2：安全生产许可证动态考核信息填报手册）。
（三）现场核查。施工现场核查由工程项目所在区县监督机构负责。按照《山东省建筑施工企业安全生产许可证动态考核办法》施工现场相关检查要求，对本地建筑施工企业在济在建工程项目进行施工现场安全生产条件核查，主要依据《危险性较大的分部分项工程安全管理规定》（住房城乡建设部令第37号）、《建筑施工安全检查标准》（JGJ59-2011）等法规、标准要求，突出危大工程安全管控、安全管理人员到岗履职、特种作业人员持证上岗、风险隐患排查整治等重点内容。各区县监督机构根据检查情况填写《建筑施工企业现场核查情况汇总表》（见附件3），于2022年11月30日前报市住房城乡建设局。
（四）结果核定。统筹企业一般安全生产条件和施工现场安全生产条件确定动态核查结论。企业一般安全生产条件或施工现场安全生产条件不合格的，即认定核查结论为不合格。
四、结果运用
考核工作结束后，市住房城乡建设局将对考核结果进行公开通报。
（一）考核基本合格企业。列入黄色监管，给予警告，在企业信用评价中给予扣分，责令限期整改，经整改仍未达到规定要求的列入红色监管。
（二）考核不合格企业。列入红色监管，企业办理安全生产许可证延期时按照延期重审办理，责令限期整改，经整改仍未达到规定的报省住建厅建议暂扣安全生产许可证。
